Initial Exam
The initial exam is given to you at No Charge because this is usually a short explanatory meeting to start your orthodontic process.
Consultation Fee
If you decide to proceed with orthodontic treatment after the consultation appointment, a total treatment fee is established and appointments are set up to begin orthodontics. Occasionally an individual desires a second opinion or decides not to proceed with treatment at this time. If this is the case, a consultation fee of $100 plus any laboratory charges must be paid to this office before any diagnostic material can be released. Should you return to our office for treatment, this consultation fee will be applied to the total treatment fee already established. (The quoted treatment fee is valid for 6 months, after which, Dr. Shiraki reserves the right to adjust the fee to current levels).
Total Treatment Fee
The total treatment fee is determined by the severity of the problem and the difficulty of the correction. The fee will vary from case to case. The total treatment fee covers the laboratory costs for before and after treatment study models and all appliances (braces, headgear, retainers) and adjustments necessary to complete orthodontic treatment. Should any of the appliances become broken or lost through patient neglect or carelessness, a separate replacement charge may be assessed. The total treatment fee covers all services performed at our office. Any fees incurred outside our office for x-rays, photos, or extractions are considered the patient’s responsibility.
